FIXED TERM CONTRACT VACANCY

July 2021 – September 2021

R4 333.00 p.m.

The AIDS Foundation of South Africa (AFSA) is a Durban-based NGO that acts as an interface between Donors and Community Based Organisations (CBO’s) working in the HIV/AIDS sector by placing donor funds with strategically selected CBOs, in South Africa, and providing them with ongoing, technical support & capacity building.

The HIV Prevention Department is made up of a number of programmes that collectively aim to collective prevent the evolution of the HIV epidemic in South Africa. AFSA wishes to appoint suitably qualified individuals to fill the roles of Child and Youth Carework (Community Care Worker).

Purpose of job:

  • Facilitate the enrolment and case management of beneficiaries at community and/or facility level.

Roles and responsibilities include but are not limited to:

  • Facilitate the enrolment and case management of beneficiaries at community and/or facility level
  • Facilitate referrals of eligible CLHIV to required services
  • Facilitate referral and linkages for uptake of HTS, ART, SRH, GBV/VAC, VMMC etc.
  • Facilitate evidence-based interventions (only trained facilitators)
  • Facilitate referrals of eligible 9-14 yrs. old to DREAMS or Comprehensive Interventions
  • Facilitate referrals to GBV/VAC cases to Social worker
  • Facilitate evidence-based interventions (only trained facilitators)
  • Support beneficiaries with clinic appointments as necessary, e.g. Plans for treatment pick-ups, fast track consultation on review dates especially for treatment failing patients
  • Complete case management tools and MER documentations
  • Confirm capturing in CBIMS
  • Liaise with Clinical Linkage Officer to enrol ALHIV into Vhutshilo 3 groups
  • Collaborate with DSPs to ensure that all ALHIV consistently attends adherence clubs and Vhutshilo 3 at facilities
  • Roll out Vhutshilo 3 sessions (14) to all ALHIV 14-18 years in the program
  • Complete case management documents for all ALHIV participating in Vhutshilo 3 and submit for capturing
  • Participate in campaigns at facility and community level

Minimum requirements:

  • Grade 12 with tertiary education recommended
  • Bachelor’s Degree in Social work Field
  • Auxiliary Social Worker/Nurse Advantage
  • CYCW Qualification Advantage
  • Counselling course
  • minimum of 2-3 years’ relevant experience
  • Facilitation skills and able to work independently
  • Report writing and interpersonal skills
  • Strong administrative and planning skills
  • Ability to work under pressure
  • Ability to work independently and as part of a team
  • Driver’s License
