KEY PERFORMANCE AREAS
. Undertake timely and accurate capturing of data
Collect patient clinical charts, registers for data capturing from clinicians, consulting rooms and wards.
Prepare, compile and sort documents for data capturing.
Capture data from available records into the required formats, eg. Databases (Tier.net, webDHIS) tables and spreadsheets.
Update the checklist of data collected and captured.
Export data between different databases when required.
Capture Antenatal HIV survey data on webDHIS when required.
Compile standard, weekly, monthly and quarterly reports with data obtained from Tier net and webDHIS systems relevant to the institution.
Verify the integrity of data
Check and verify the integrity of data received from clinicians, consulting rooms and wards, by comparing previously submitted reports and statistics and with source documents.
Collect data from all sections on a daily basis.
Check the completeness of webDHIS data from the sections to ensure that all data elements are reported on.
Check the accuracy of data and correct incorrect data where necessary.
Check and query missing information and errors observed during data entry.
Obtain further information for incomplete documents. Follow up for non-submission of data from various sources.
Provide information to components
Compile daily, weekly, monthly and quarterly reports.
Generate and print reports (weekly, monthly, and quarterly) timeously and submit to the facility manager
Submit the list of early, late missed appointments and loss to follow-up clients (defaulters) to the facility manager (or HAST champion)
Distribute reports to the relevant managers in the institution.
Submit the reporting templates and dispatch files to the mother institutions for daily, monthly and quarterly reporting.
Report all challenges and problems at the earliest opportunity.
Retrieve data from the databases or electronic files as requested.
Liaise with different sections to collect data and information in the correct formats and within agreed timeframes
Attend data-related meetings and give feedback to the management team.
Secure captured information in accordance with system specifications and protocols.
Perform regular backups to ensure data preservation.
Update registers and statistics on a regular basis and delete unnecessary files.
Maintain proper filing and safekeeping of data records.
File completed documents in designated locations. Comply with data integrity and security policies.
Note: successful applicants would be expected to also work over the weekends, night shifts, and sometimes extended hours during the week where applicable.
