Grade 12 certificate with knowledge or skills to work with/in the community. Ability to walk by foot on daily basis visiting households to take care of their health needs. Good communication skills: verbal, listening, reading and writing. Able to speak any of languages of the local communities they will be serving. Ability to handle confidential information. Good interpersonal skills (work independently and in a team). Prepared to work over weekends and public holidays when need arises. Training in Home Based care (69 days) or Phase One will be an advantage. Knowledge of Batho Pele Principles and Patient Rights Charter. No criminal record.
Promote overall health and well-being within households and communities. Provide information, promote healthy life style and disease prevention among communities through health education. Conduct structured household screening and profiling to identify health needs. Provide appropriate direct basic services including treatment for minor health problems/needs, counselling and psychosocial support for individuals or households. Facilitate appropriate referral for health and social support services as needed for individuals or households. Provide adherence support for patients on medication and support follow-up care, including delivery of chronic medication. Tracing of patients who missed or defaulted their appointments for collecting medication. Facilitate community mobilisation and create awareness on disease prevention through community campaigns.
